By signing and agreeing to the Photo/Video/Testimonial Release (together with these Terms & Conditions, the  “Release”), I hereby irrevocably grant to CCC Intelligent Solutions Inc. (“CCC”) and its parent, affiliates subsidiaries, related companies, advertisers, successors and assigns the right and permission to use in perpetuity, my name, likeness, image, and appearance (“Likeness”) in any manner and in any media, including, but not limited to, publicity materials, press releases, websites, advertisements, training materials, videos and product literature, now known or later developed (the “Material”), throughout the world, at any time, without review, permission or compensation of any amount or kind. CCC shall have complete ownership of any Material containing or featuring my Likeness, including copyright interests, and I acknowledge I have no interest or ownership in the Material (or any portion thereof) or its copyright. I understand that CCC has the right, but not the obligation, to use my Likeness.

I agree that CCC may use my Likeness in other future Material without obtaining another Release and that the terms of this Release shall apply to all such uses. In addition, I agree that my Likeness may be used in whole or part and may be retouched, enlarged, reduced or otherwise altered provided that it remains true to the actual subject matter. I waive the right to inspect or approve any Material or any use to which my Likeness may be put.

I release and discharge CCC, its parent, affiliates, subsidiaries, related companies, advertisers, successors and assigns and each of their officers, directors and employees from all claims, causes of action, suits, costs, demands, liabilities or judgments whatsoever, in law or in equity, against CCC arising out of or in connection with the use of my Likeness. I agree to indemnify and hold harmless CCC its parent, affiliates, subsidiaries, related companies, advertisers, successors and assigns and each of their officers, directors and employees from all claims, suits, costs, demands, liabilities, judgments and causes of action, in law or in equity, which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of the grant contained in this Release.

This Release constitutes the entire understanding and agreement with respect to the subject matter hereof and cannot be amended except by a written instrument signed by the parties hereto. This Release will be governed by the laws of the State of Illinois without giving effect to conflict of laws provisions.

By agreeing to the Release, I warrant that I am of legal age, have every right to contract in my own name and have not granted the exclusive right to use my Likeness to any other person or entity.

THIS RELEASE PROVIDES CCC WITH MY ABSOLUTE AND UNCONDITIONAL CONSENT, WAIVER AND RELEASE OF LIABILITY, ALLOWING CCC TO PUBLICIZE AND COMMERCIALLY EXPLOIT MY LIKENESS AS SET OUT ABOVE. BY AGREEING TO THIS RELEASE, I ACKNOWLEDGE THAT I HAVE COMPLETELY READ AND FULLY UNDERSTAND THE RELEASE AND AGREE TO BE BOUND THEREBY.
